**Service Support Worker – Plymouth
Location: Derriford Hospital (requires travel throughout the surrounding areas)
Contract type: Fixed term contract until 30th June 2022
Hours per week: 35 (flexibility required as we have a range of shift patterns between 9am – 5pm and are keen to discuss these with potential candidates)
Salary: £16,914 – £19,701 per annum pro rata **

We are looking for a passionate and enthusiastic person with great people skills to join our Independent Living team to support our surge response.
At the British Red Cross, we put the people that need us at the heart of what we do. As a Service Support Worker, you will provide the support and care which can make a vital difference to the quality of people’s lives, allowing them to continue living at home in comfort.
You will provide reactive, focused and flexible support to adults following a hospital stay to enable early discharge when clinical needs have been met. You will assist the Independent Living Service Coordinator by undertaking duties required to meet the service users’ needs in or der to re-enable service users to remain in their own home and prevent hospital re-admission.
You will respond to referrals from Clinicians and Health Professionals and will be required to have the ability to assess service users’ needs, complete and follow a support plan for the individual. In addition, you will be providing practical and emotional support in the service user’s own home and signposting to other services where appropriate to enable the individual to live independently in their own home.

The Independent Living (IL) & Crisis Response (CR) service (IL/CR) supports people in t he space between hospital and home and responds to the needs of people in crisis following an emergency; maintaining an extensive network of external relationships across health, social care and emergency services.
This role could be for you if…

Also, you should be the following:
- Educated to GCSE level (or equivalent by experience)
- IT literate
- Full manual transmission driving licence holder and ideally have access to your own vehicle (you must also have, or be willing to obtain vehicle insurance for business use)
- Have a good knowledge of services provided by the NHS and Social Care
This role requires use of full Personal Protective Equipment, including Fluid resistant face masks, to be worn at all times while delivering services. This will be provided for you.
The role will involve frontline work with vulnerable service users and will not always allow for a 2-metre social distance to be maintained. You will be asked to complete a Risk assessment to confirm you are able to take up Frontline work, at onboarding stage. If you consider yourself to be a person at greater risk from Covid or you are unable to wear a facemask for long periods of time then this role may not be suitable for you. In this role, there may be a requirement to enter care homes or NHS premises. Due to recently introduced legislation, any workers entering a care home environment must be fully vaccinated against COVID-19 (including visiting services). From April 202 2 this applies to all regulated services and we also expect that the same will apply to those directly supporting the NHS, for example those working on NHS premises. Therefore applicants must be fully vaccinated with a UK approved vaccine and we will ask to see evidence of vaccination status or exemption as part of the recruitment and onboarding process.
